Statutory Information Checklist —

See below our list of forms so that you can provide the legally-necessary Statutory Information throughout the build process. Without this, we cannot complete the necessary functions.


Form 1 – Quote


This form is used to obtain a quotation, this information together with plans and details if available.

  • All applications require a written quote from us with terms and conditions and a copy of the contract, without these we cannot submit an Initial Notice and Insurances would not be valid.
  • Once a quote is accepted by or on behalf of the accepting Client, Form 2 need submitting.
  • This MUST be a minimum of 5 working days before you start the building works on site in ALL cases.

Form 2 – Building Regulations Application


This form is used to confirm all the details are correct and agree to us serving the Initial Notice, this is a legal document and MUST be served 5 working days before the building work starts on site.

  • Without this we cannot proceed
  • This is your chance to ensure the information provided is legally correct and accurate.
  • There are several questions that legally must be answered, information provided, and please note it is the Clients duty to ensure all is correct.
  • Amendments required after this may require an additional administration fee to be paid.

Form 3 – 2 Day Start Notification


This form is used to notify us of your planned start on site date, this is when the builder is due to start on site, as you want us to commence inspections – you are legally obliged to give a minimum 2 working days’ notice of works starting, if known at the time of submission then the date off the application form will be recorded.

Form 4 – 5 Day Commencement Notification


This form is used to notify us when the work reaches the point of commencement. You are legally obliged to give notice within 5 working days’ notice of works reaching this point.

  • For new buildings and extensions this will generally be when reach dpc level so generally blue brick height, oversite level;
  • For other building works at the point 15% of the works is reached;
  • If no commencement has been notified that after 3 years the Initial Notice automatically ceases to be in force and a new application will be required, (if regulations have changed this may involve a redesign and additional fee for checking etc).

Form 5 – Change of Duty holders or addition of a new duty holder


This form is used to remove or to add or remove duty holders.

  • Example Clients duties: A domestic client is any individual who has building work carried out on their home, or the home of a family member. The building work is done for domestic purposes, not for business purposes. As the domestic client, you are in control of the project, which means you must for example:
    – allocate enough time and resource for the building work to comply with building regulations.
    – appoint designers and contractors.
    – co-operate with anyone working on, or in relation to the project, so everyone involved can comply with their duties and functions.

Please remember this is your project, so choose wisely.

Form 6 – Amendment Notice


This form is used to where the work on the Initial Notice has changed and therefore the description needs changing.

  • Additional work (may need a quote for additional fees).
  • Reduction in work.
  • A more accurate description required.

Please remember this will appear on all legal paperwork so must be accurate, if not may create issues when selling or mortgage changes take place. (once a final certificate is issued this cannot be amended)

Form 7 – Client Name Change Request


This form is used to used where the client detail need updating. For example:

  • Change of owner;
  • Correction of details supplied previously.

Form 8 – Regulation 38 Statement


This form is used to when the Fire Safety Order (FSO) applies, (most premises other than those that are serving as a shared dwelling) to confirm a declaration that:

  • in accordance with Regulation 38 of the Building Regulations 2010 (as amended) all the information shown above is fully understood, care has been taken to prepare accurate information which has subsequently been passed onto the person responsible for fire safety to allow them to manage the building effectively.
  • We have also received a notice from the responsible person acknowledging receipt of the fire safety information and confirming the information provided is sufficient to enable them to understand, operate and maintain the building (and the fire safety systems in it) after the building work in question.
  • AD B Volume 2 2-19.3 and 19.4 provide basic detail required
  • For complex building see AD B Volume 2-19.5-19.6 and BS9999 Clause 9 and Appendix H as more detail is required

Form 9 – Final Compliance Declarations (Client)


This form is used to confirm a declaration in accordance with the regulations that:

  • as Client all building works are complete;
  • as Client that to the best of my knowledge “the works comply with the applicable requirements of the Building Regulations”;
  • This should not be submitted until you can confirm this statement;
  • Failure to submit the statement means a final certificate cannot be issued.

Form 10 – Final Compliance Declarations (Contractor)


This form is used to confirm a declaration in accordance with the regulations that:

  • as Contractor all building works are complete.
  • as Contractor that to the best of my knowledge “the works comply with the applicable requirements of the Building Regulations”.
  • This should not be submitted until you can confirm this statement.
  • Failure to submit the statement means a final certificate cannot be issued.

Form 11 – Final Compliance Declarations (Designer)


This form is used to confirm a declaration in accordance with the regulations that:

  • as Designer all building works are complete.
  • as Designer that to the best of my knowledge “the works comply with the applicable requirements of the Building Regulations”.
  • This should not be submitted until you can confirm this statement.
  • Failure to submit the statement means a final certificate cannot be issued.

Form 12 – Regulation 40B Notification


This form is used to confirm a declaration in accordance with the regulations below have or will take place:

Information about overheating

40B.

  1. This regulation applies to building work in respect of a building where Part O of Schedule 1 applies where Part O of Schedule 1 imposes a requirement in relation to building work.
  2. The person carrying out the work must, not later than five days after the work has been completed, give sufficient information to the owner about the provision made in accordance with Part O so that the systems in place further to Part O can be operated in such a manner as to protect against overheating.
